6.Access your performance review form

    Introduction

    You can access any of the forms, current or previous, which a manager has uploaded to iTrent following a performance review discussion with you.

    Follow the steps to view and download your performance review form.

    Open iTrent

    Use the button below to open the application. Then log in using your username and password.

    Please note:

    We advise you to use Microsoft Edge or Google Chrome to access employee self-service (iTrent) as other browsers may cause issues.

    Find the review form

    Select Performance from the left menu.

    Make sure the All button is selected.

    Select the form you would like to view.

    View or download the appraisal

    You can now either select the:

    • Start button to view the form
    • Print button to download a PDF version of the document. It may take a few minutes for the PDF to download.

    To return to your list of forms, hit the Back to Reviews button in the top left corner.

    Body composition scan (Boditrax)

    We’ll ask you to step onto our Boditrax machine, which takes 30 seconds to complete a full body composition scan.

    This will provide you with 15 health metrics including:

    • visceral fat
    • hydration levels
    • metabolism
    • bone mass.

    Blood pressure

    We’ll wrap an inflatable cuff around your arm. When it inflates it’ll tighten on your arm, and provide a blood pressure reading.

    This reading will tell us whether your blood pressure is healthy, or if it’s too high or low.

    High blood pressure can increase your risk of developing serious problems, such as heart attacks and strokes, if it’s not treated.

    Low blood pressure is not usually a problem, however it can cause dizziness and fainting in some people.

    Peak flow assessment

    You’ll use a peak flow meter to measure how fast air comes out of your lungs when you exhale forcefully.

    Your peak flow readings can help to recognise early signs of asthma.

    Diabetes prevention assessment

    We’ll use the ‘Know Your Risk’ tool to assess your risk of developing type 2 diabetes.

    It asks 7 simple questions related to:

    • age
    • gender
    • ethnicity
    • family history
    • waist measurement
    • body mass index (height and weight)
    • blood pressure.

    Based on your answers, you’ll be given a score.

    Using a points-based system, you’ll also be able to see if you have a low, medium, moderate or high risk of diabetes.

    We’ll discuss with you any actions you may benefit from, such as lifestyle changes or a GP referral.

    Cholesterol check

    We can also carry out a cholesterol check if:

    • you are aged 40 to 74
    • have not had an NHS health check in the last 5 years
    • do not have a pre-existing condition.

    The results of a cholesterol check can tell you your risk of getting heart disease or suffering a stroke.

    Mental health screening and support

    This is an optional service that helps to determine whether you are experiencing symptoms of a mental health condition, and receive advice on the support that is available, both at work, and in your local community.
