Introduction
You can request a DSE eyecare voucher if you:
- are a DSE user
- have not had an eye test in the last two years.
You can use the voucher to receive:
- a free eye test
- free glasses for DSE use only (from the £50 range)
- money towards your glasses.
How to request a voucher
Follow these steps to request a voucher:
1Check that you do not qualify for an NHS voucher
You may qualify for a free eye test and optical voucher from the NHS.
Please check this before continuing with this request:
2Talk to your line manager
If you'd like to take advantage of this scheme, talk to your line manager who will be able to confirm if you are classed as a DSE user.
3Book your eye test
To book an appointment you can:

4Fill in the online form
Connect to the council network using a desktop computer or laptop (this form will not work with council-issued mobile phones).

What you'll need to provide
The form will ask you to provide your:
- contact details
- payroll number (you’ll find this on you payslip)
- appointment details
- delivery method for the voucher.
5Manager approval
Your completed form is automatically sent to your line manager so that they can confirm you qualify for a free eye test.
If they are the budget holder for your area they will also be asked to enter your service area cost code.
If they are not the budget holder, they will be asked to enter the budget holder's details.
The form is then automatically sent on to the budget holder for them to confirm that they are happy to pay for the voucher.
6Ordering your voucher
Once your manager has approved your request, the form is then sent to the corporate support team. They will check your details and order your voucher.
7Receive your voucher
Your eVoucher will either be sent to:
- you by email directly from Specsavers - this can be downloaded to your device and printed off
- your line manager to print off and give to you
- you by post, in exceptional circumstances only, at your manager's discretion.
You will have specified your preferred method in the online form.
8Confirm you have received the voucher
